Spectrum Health Care is hiring a Registered Practical Nurse (RPN) as a full-time Home Support Supervisor to work with clients in Mississauga and surrounding areas. You will lead a multidisciplinary team in the Integrated Care Program, building healthier communities through personalized, compassionate care.

The role requires at least three years of leadership experience in a community setting, Ontario CNO registration, and a valid driver’s license.

Formación

• Current registration with the Colleges of Nurses of Ontario (CNO) in good standing.
• Minimum three years’ experience with progressive leadership responsibilities as a direct supervisor, preferably in a community setting.
• Valid driver’s license and access to a reliable vehicle.
• Excellent English verbal and written communication skills; additional languages are a plus.

Responsabilidades

• Travel to patients' homes in the community and collaborate with multidisciplinary teams.
• Assess patient needs, formulate the plan of care, and monitor progress toward outcomes.
• Manage staff performance, conduct appraisals, and implement progressive disciplinary action as needed.
• Collect and analyze feedback and key indicators such as caseload, turnover, satisfaction, and risk.
• Address and document patient concerns and complaints in a timely manner.
