Job summary

ParaMed Home Health Care in Windsor, Ontario is seeking a Registered Practical Nurse to deliver scheduled community visits under RN supervision. You will care for patients of all ages in various home settings, promoting comfort and independence while balancing autonomy with safety.

Qualifications

  • Registration with the CNO as a Registered Practical Nurse in good standing.
  • Valid driver’s license and access to a vehicle.
  • Experience in delivering community care is an asset.
  • Willingness to travel within a designated area.

Responsibilities

  • Participate in scheduled community visits to look after patient needs within your scope of practice.
  • Provide ethical and safe nursing care.
  • Use appropriate assessment tools/techniques to assess patients’ individual needs.
  • Execute and continuously monitor care plans and adjust as required.
  • Engage patients using a collaborative approach by building rapport and trust.
  • Maintain and update professional patient records related to patient encounters and plans.

What You’ll Do
