Job summary

ParaMed Home Health Care is seeking qualified RNs or RPNs to provide in-home patient care across Ontario. You will perform assessments, implement and monitor care plans, and build trusting relationships with patients and families in their homes.

The role emphasizes autonomy, ongoing training, and collaboration with a supportive nursing team. Eligible candidates should have CNO RN registration, a valid driver’s license, and a willingness to travel within the designated area.

Qualifications

• Registration with the CNO as a Registered Nurse in good standing.
• Valid driver’s license and access to a vehicle are required.
• NSWOC certification is an asset.
• Experience delivering community care is an asset.
• Reliability and flexibility are essential.
• Willingness to travel to patient residences within a designated area.

Responsibilities

• Using appropriate assessment tools to assess patients’ individualized needs.
• Executing and continuously monitor care plans.
• Engaging patients with a collaborative approach by building rapport and trust.
• Maintaining and updating professional patient records related to patient encounters and plans.

In this community nursing role, as a Registered Nurse (RN) or Registered Practical Nurse (RPN) you will have scheduled visits to care for patients in various settings. You will work with patients of all ages with differing complexity of care needs and play a key role in helping patients remain comfortable in their homes for as long as possible.
