Job summary

Alberta Health Services is seeking a Transition Coordinator RN to support patients and families as they navigate care decisions. You will draw on information from multiple sources to help them move safely and confidently to the right place of care, balancing clinical insight with system-wide coordination.

The role is fast-paced and collaborative, with emphasis on problem-solving, PFCC, and complex discharge management.

Qualifications

• Completion of an accredited nursing education program.
• Active or eligible for registration with the College of Registered Nurses of Alberta (CRNA).
• Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification.
• Five or more years’ experience in good standing with CRNA.
• Experience performing comprehensive assessments of individuals within their physical and social environments.
• Skilled in collaborative practices and concurrent care planning.
• Experience with PFCC and Complex Discharge Management.
• Solution-focused and able to navigate difficult conversations.
• Able to tolerate high levels of stress and deadlines.
• Able to lift 10-15 lbs and walk up to 3 km per day.
• Hands-on care during assessment (transfers, bariatric clients, etc.).
• Preferred: RN with Home/Community Care experience and PFCC.

Responsibilities

• Provide nursing services to patients, families, communities and populations.
• Lead integrated, holistic patient care and health promotion.
• Support transitions and discharge planning across care settings.
