Detailed Overview

In accordance with the Mission, Vision, and Values of Fraser Health and the British Columbia College of Nurses and Midwives (BCCNM) standards of practice, the Registered Nurse/Registered Psychiatric Nurse (RN/RPN), Mental Health & Substance Use Services (MH&SUS) works collaboratively and in partnership with the interprofessional care team in the provision of person-centered and recovery-oriented care. Provides treatment and care management services that are both inclusionary and flexible for individuals with mental illness and concurrent disorders (e.g., substance use, developmental disorders) to meet client needs and ensure integration and continuity across the care continuum. Works collaboratively within an interdisciplinary team-based model of care and promotes rapid engagement in the delivery of timely, relevant, and client/family-centered care. Emphasizing recovery and rehabilitation, reducing barriers to care, and promoting a culturally safe, trauma-informed approach, the nurse establishes a welcoming therapeutic environment and promotes client self-determination and independence.

Responsibilities
  1. Coordinates care for clients by collaborating with the interprofessional care team, performing intake functions, conducting mental health assessments, developing, implementing, maintaining, and adjusting comprehensive treatment plans based on client needs and best practices, administering prescribed medications, and monitoring clients' mental state, response to medications, and treatment efficacy to ensure optimal resource use and client-centered care.
  2. Plans and provides direct nursing care to clients in compliance with standards of practice by reviewing client/family history, conducting assessments, assessing vital signs, developing care plans, and collaborating with other healthcare team members. Implements treatment modalities such as ADLs, wound care, and medication administration as required.
  3. Works from a strengths-based model, utilizing motivational interviewing and engagement techniques, collaborating with clients and professionals to develop and implement individualized care plans, and assisting clients in accessing community services.
  4. Establishes, maintains, and enhances therapeutic relationships based on respect with clients and their support networks by engaging stakeholders, providing support and follow-up, promoting self-care, and empowering clients to improve their quality of life. Educates clients and families about management and treatment of psychiatric conditions and substance use issues.
  5. Provides treatment services including crisis intervention, individual counseling, and education, using various theoretical models to address clients' concerns, goals, and learning needs.
  6. Participates in case conferences and team meetings for coordinated care, acts as a client advocate, and ensures clients' rights to autonomy, respect, privacy, and access to information are upheld.
  7. Collaborates with the care team, including physicians, primary care providers, and other health professionals, to address issues, modify care plans, and respond to changes in client condition.
  8. Monitors Extended Leave under the Mental Health Act, ensuring compliance and continuity of care.
  9. Assists clients in accessing services such as medical appointments, hospitals, and social programs, including providing information, assisting with forms, transportation, and daily tasks.
  10. Documents assessments, care plans, interventions, responses, outcomes, and referrals according to standards and procedures.
  11. Ensures a safe environment by adhering to safety practices, reporting risks, and anomalies.
  12. Identifies personal learning needs, discusses goals with the Team Coordinator, and engages in activities to meet these needs.
  13. Participates in research and quality improvement activities by collecting data and recommending practice improvements.
  14. May be required to deliver educational sessions, facilitate group sessions, provide outreach nursing care, and collaborate with residential and community agencies.
  15. Performs other related duties as assigned.
Qualifications
Education and Experience

Graduation from an approved School of Nursing or Psychiatric Nursing. Current registration with the British Columbia College of Nurses and Midwives (BCCNM). At least one year of recent, relevant experience in mental health and/or concurrent disorders in community or acute settings. May require completion of a substance use certificate program. Valid BC Class 5 Driver's License and access to a personal vehicle with appropriate insurance.
