Job summary

VHA Home HealthCare in Ontario seeks an experienced Registered Nurse to join the Child and Family Program serving Central & Central East. You will deliver home-based nursing care to newborns through young adults up to 25, coordinating care with families and the broader health team.

As a visiting nurse, you will assess needs, develop and monitor care plans, document thoroughly, and uphold College of Nurses of Ontario practice standards.

Qualifications

• RN in good standing with the College of Nurses of Ontario (CNO).
• Membership in a professional association (RNAO) preferred.
• RN diploma/degree from a recognized university or college.
• Valid First Aid or CPR Certification.
• Wound care or venipuncture knowledge is an asset.
• Acute Care and Medical-Surgical nursing experience is an asset.
• Knowledge of community nursing is an asset.
• Demonstrated communication and documentation skills.
• Reliable attendance, punctuality, and availability for the role.
• Valid Ontario driver's license (G) and a reliable vehicle are required.

Responsibilities

• Assess, monitor, and coordinate care planning.
• Evaluate care plan interventions through reassessment and evaluation.
• Ensure quality of care by addressing factors affecting practice autonomy.
• Demonstrate professionalism, leadership, and accountability in client/family interactions.
• Support client safety through quality improvement principles.
• Inform nursing supervisor of client activities and changes, and document to the case coordinator.
• Maintain documentation standards per the College of Nurses of Ontario.

Pay Range

Visit Rate: $33.53 - $41.10 /visit (placement on salary range as per Collective Agreement)
Estimated annual earning potential: $80,000–$130,000. Actual earnings may vary based on availability and service volumes

Grounded in our commitment to Client and Family Centred Care the key foci on the RN role is to provide and coordinate home health nursing services to diverse group of individuals, families and groups according to the College of Nurses of Ontario Practice Standards, the policies and procedures of VHA, the program where they provide services, and the Standards of Practice in Community Health Nursing.

The Child and Family Program provide individualized, one-on-one care to clients ranging from newborns to young adults up to 25 years of age. As a visiting nurse, you will deliver home-based nursing care to children with complex medical needs, supporting both their health and overall well-being in a family-centered environment. The program also supports continuous professional development through educational opportunities, including access to the SickKids Connected Care program, enabling nurses to enhance their clinical knowledge and stay current with best practices.

Key Areas of Accountability

• Assess, Monitor, and Makes Care Planning/Care coordination
• Evaluates care plan interventions through on-going reassessment and evaluation results.
• Ensures the quality of care of patients by identifying and acting on factors which affects practice autonomy and delivery of quality care.
• Demostrates professionalism, leadership, judement, and accountability in interactions with client and families.
• Supports client safety care through the development of knowledge and skills in accordance with our principles of continuous quality improvement.
• Informs nursing supervisor of any client activities and relevant concerns or changes in the client or the client care plan and informs/submits appropriate documentation to the case coordinator.
• Maintains documentation standards according to the College of Nurses of Ontario.

Required Qualifications

• A moderate level of wound care or venipuncture knowledge and experience is an asset.
• Acute Care and Medical-Surgical nursing experience is an asset.

• Demonstrate effective communication (written and verbal) and documentation skills.
• Demonstrate reliable attendance, punctuality, and availability for the role.
• Commitment to modeling lifelong learning and professional growth and can apply critical thinking skills to provide client care.
• A valid Ontario driver's license (G License) and a reliable vehicle for traveling are required.
