The position supports refugees through the Refugee Health – Coordinated Access Program, working with diverse populations and ensuring safe, quality care across the continuum.

Qualifications

• 3–5 years nursing experience in community/public health settings.
• Experience working with refugees and diverse communities.
• Knowledge of area policies and programs affecting practice.
• Ability to engage sensitively with vulnerable populations.
• Understanding of community resources and navigation.
• Strong communication and interpersonal skills; organizational abilities.
• Experience in program development and evaluation.
• Valid Class G driver’s license and reliable transportation.

Responsibilities

• Support the mission and ensure safety and quality care for clients/patients.
• Act as a resource and advocate for refugees and communities.
• Build partnerships with internal teams and community organizations to support coordinated access.
• Participate in program development, implementation, monitoring and evaluation.
• Maintain confidentiality and chart/documentation integrity.
• Collaborate with primary care providers and other health professionals.
