Job summary

Dundurn Place Care Centre in Hamilton, Ontario is seeking a Social Service Coordinator to join our team. The role reports to the Executive Director and focuses on orientation, informal education, support and counselling to residents, families and staff to promote comfort and empowerment.

The ideal candidate will have a diploma or degree in social work, be registered, and bring excellent interpersonal and communication skills.

Qualifications

• Diploma in social services, gerontology, or a degree in Social Work.
• Current registration under the applicable Social Work Act.
• Three years' experience in a social work role within long-term care is preferred.
• Ability to support residents, families and staff with counseling and empowerment.
• Must complete Vulnerable Sector Check and health clearances.
• Bilingual is an asset.

Responsibilities

• Develop, implement and evaluate social service programs.
• Provide orientation, education and counseling to residents and families.
• Coordinate family council and outreach to hospital/social services.
• Assist with resident placement and occupancy in coordination with LHIN targets.
• Lead support groups and bereavement counseling.
• Participate in recruitment, orientation, retention and performance management.
• Maintain monthly and quarterly satisfaction analyses with action plans.
