Job summary

Dundurn Place Care Centre in Hamilton, Ontario, is seeking a Social Service Coordinator to lead resident support initiatives and coordinate family services. The role emphasizes compassionate care, teamwork, and community engagement to promote well-being and empowerment within the home.

The position reports to the Executive Director and focuses on orientation, education, and counseling for residents, families, and staff, ensuring high-quality care and strong inter-personal relationships across

Qualifications

• Diploma in social services, gerontology, or a degree in Social Work.
• Registered under the Social Work and Social Services Work Act with current general certificate.
• Three years of full-time experience in social work/senior care within past five years may apply.
• Knowledge of quality indicators and ability to improve them.

Responsibilities

• Develop, implement and evaluate Social Service Programs.
• Provide support to department managers in staff intervention when needed.
• Facilitate support groups for families and/or residents.
• Bereavement counselling for residents/families/staff as needed.
• Coordinate Family Council meetings.
• Act as Home Resident Placement Coordinator.
• Interact with residents, families and staff to promote well-being.
• Maintain relationships with hospitals, agencies, and referral sources.
