The Resident & Family Experience Coordinator III plays a critical role within our communities, impacting residents’ lives and fostering human connection.

As a Resident & Family Experience Coordinator III, your typical day will include:

  1. Leading the resident move-in, transfer, and move-out process in line with organizational standards, including facilitating applications, assisting with financial supports, conducting tours, and introducing residents/families/SDMs to the community and its operations.
  2. Supporting residents and families through social work counselling, collaborating with the inter-professional team to assess and develop care plans that address psychosocial and social needs, promoting overall well-being and safety.
  3. Planning, developing, implementing, and evaluating social work services and support groups tailored to residents’ and families’ needs, engaging external resources as necessary.
  4. Assisting residents and families with advanced care planning and treatment discussions within the scope of Social Services, including funeral arrangements.
  5. Recruiting and supervising social work students’ placements.
  6. Managing feedback received in the resident home area/neighborhood, ensuring timely responses and satisfactory outcomes.
  7. Providing reports and analysis of feedback to Leadership & Quality Committees.

Must haves:

  • Registered Social Worker as per provincial legislation.
  • Minimum of three years’ experience in healthcare, social services, or related sectors.
  • Knowledge of the Long Term Care Act & Regulations (preferred).
  • Customer service experience (asset).
  • Hospitality and/or sales experience (asset).
  • Strong oral and written communication skills.
  • Organizational, planning, and delegation abilities.
