Alberta Health Services – Alberta Kidney Care North in Grande Prairie is seeking a Social Worker II to join an inter-disciplinary team. You will provide psychosocial assessments, counseling, and resource navigation for patients with kidney disease and their families within a dedicated Renal Social Worker II unit.

The role includes risk screening, discharge planning, and collaboration with health professionals to ensure holistic patient care.

Formación

• Bachelor's degree in Social Work required; eligible for ACSW registration.
• 1–2 years' experience as a renal social worker or with chronic/life-threatening illnesses.
• Current Basic Life Support (BLS-HCP) certification.
• Strong interpersonal, organizational, and communication skills; ability to work in a fast-paced multidisciplinary team.

Responsabilidades

• Conduct psychosocial assessments and resource-based interventions.
• Provide individual, group and family counseling and support.
• Coordinate case management, transition or discharge planning, and prevention activities.
• Document care plans and collaborate across the interdisciplinary team.
