Canadian Shield Health Care Services is seeking a Registered Physiotherapist for our integrated care clinic. The role focuses on comprehensive assessments, personalized treatment plans, SMART rehabilitation goals, and coordinated referrals. Wage is based on experience.

Full virtual is available and a fully remote option is offered, with treatments and travel limited to local assignments as needed. Strong clinical skills and Ontario registration are required.

Qualifications

• Completion of a Bachelors’ or Masters’ Program in Physical Therapy from recognized Canadian or International University.
• Current registration in good standing with the College of Physiotherapists of Ontario.
• Proof of professional liability insurance.
• Active ADP license for mobility aids or intent to obtain ADP license from MOHLTC.

Responsibilities

• Conduct comprehensive client assessments and develop customized treatment plans.
• Create SMART, patient-centered rehabilitation goals, provide care planning, and coordinate referrals.
• Deliver physiotherapy interventions to improve strength, mobility, balance, and function.
• Educate clients and caregivers on safe mobility techniques, exercise programs, and prevention strategies.
• Monitor progress, update care plans, and maintain accurate documentation in compliance with standards and policies.
