Job summary

County of Simcoe seeks a Nurse Practitioner to join our long-term care home team. You will provide comprehensive health assessments, diagnose and treat residents, and collaborate with the Most Responsible Practitioner to coordinate care.

Candidates should have 2–3 years NP experience with a geriatric focus, strong documentation, and the ability to work days/evenings/weekends. Ontario RN-EC registration is required; valid driver’s license is an asset.

Qualifications

Baccalaureate Degree in Nursing is required.
Nurse Practitioner Diploma in Primary Care or Adult is required.
Current Ontario RN-EC registration with the College of Nurses of Ontario.
Affiliation with RNAO or NPAO with PLP or equivalent.
Post-graduate NP certification in Palliative Care or Gerontological care preferred.
Valid G class license and access to a vehicle.
Criminal Records Check and vaccine proof required.
Evenings and weekends availability may be required.

Responsibilities

Practice per provincial legislation and standards.
Collaborate with home interprofessional team for resident-centred care.
Engage in evidence-informed practice and continuous professional development.
Lead and mentor nursing colleagues and contribute to standards and quality initiatives.
Perform health assessment, diagnosis, and therapeutic management including prescribing.
Provide education and support to residents, families and care teams.
Ensure safe documentation and compliance with privacy laws.
