Job summary

Stem Health seeks a Registered Social Worker to join our mental health team at the First Canadian Place flagship clinic. You will provide evidence-based care within a time-rich, relationship-based model, collaborating closely with a multidisciplinary team to support members’ wellbeing and proactive mental health goals.

The role emphasizes thoughtful, clinical warmth, and a long-term view of clients’ mental health journeys in a private primary care setting in Canada.

Qualifications

• Master of Social Work (MSW) from an accredited program required.
• Registered Social Worker (RSW) in good standing with OCSWSSW required.
• Minimum 3 years of post-registration clinical experience.
• Proficiency in evidence-based modalities such as CBT, ACT, mindfulness-based interventions or psychosocial counselling.
• Experience in private practice or integrated care environments is an asset.

Responsibilities

• Conduct biopsychosocial assessments for new members and establish a clinical picture.
• Provide evidence-based therapeutic interventions tailored to clients’ goals using CBT, ACT, mindfulness, and related approaches.
• Deliver time-rich in-person sessions in the flagship clinic.
• Monitor progress and adjust approaches over time.
• Collaborate with a multidisciplinary team to integrate mental health within the Unified Health Story.
