About Stem Health

Stem Health is reimagining the primary care experience for individuals who want a more thoughtful relationship with their health. Physician-led and membership-based, we combine medical excellence with the warmth of hospitality - anchored in a beautifully designed flagship clinic at First Canadian Place, opening September 2026. Our care model is relationship-based, data-informed, and built on time, trust, and exceptional service.

The Vision

At Stem Health, we believe that mental resilience is the silent engine of longevity the vital pillar that connects physical performance to a life well‑lived. In a world where mental health is too often treated as a reactive crisis or a disconnected checkbox, we are building a space where the mind is prioritized as a proactive, core component of every member's Unified Health Story. Our mental health program is an integral pillar of the Stem Health experience delivered in the same thoughtfully designed, unhurried space where members receive their physical care. As our Registered Social Worker, you are the steady, grounded presence within that space: someone who understands that the most profound shifts happen when a client feels truly seen, truly heard, and completely at ease.

Position Summary

We are seeking a skilled and compassionate Registered Social Worker (RSW) in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) to join Stem Health's mental health team at our First Canadian Place flagship location. In this role, you will provide evidence-based therapeutic care to Stem Health members, working as an integrated member of our multidisciplinary clinical team. This is not a high-volume, transactional caseload. It is a time-rich, relationship-based model where your clinical expertise, warmth, and therapeutic precision are the standard not the exception. You will work with health-engaged, high-performing members who are seeking proactive mental health support as part of a broader commitment to their wellbeing, and you will help us build and shape the mental health program that serves them.

Key Responsibilities
Therapeutic care and clinical assessment
  • Conduct comprehensive biopsychosocial assessments for new members, establishing a clear clinical picture of their mental health, social context, life stressors, and therapeutic goals.
  • Provide individualized, evidence-based therapeutic interventions tailored to each member's presenting concerns, goals, and values drawing from modalities including Cognitive Behavioural Therapy (CBT), Acceptance and Commitment Therapy (ACT), mindfulness-based approaches, solution-focused therapy, and psychosocial counselling.
  • Deliver time-rich in-person sessions within our flagship clinic that prioritize depth, presence, and genuine therapeutic relationship creating an environment where members feel as safe and unhurried as they do in any part of the Stem Health experience.
  • Monitor member progress over time, adjusting therapeutic approaches as needed and maintaining a longitudinal view of each member's mental health journey.
  • Provide therapeutic care and navigation for members navigating grief, life transitions, relationship difficulties, workplace stress, burnout, anxiety, mood concerns, and other psychosocial challenges.
Care coordination and integrated practice
  • Work as an active, connected member of Stem Health's multidisciplinary clinical team sharing relevant clinical insights within the member's Unified Health Story and ensuring therapeutic work is integrated with their broader health picture.
  • Collaborate with the primary care physicians, care coordinators, and other allied health providers on cases where mental health intersects with physical health, lifestyle, or chronic disease management.
  • Identify members who may require a higher level of psychiatric care or crisis support, and facilitate appropriate referrals and care transitions with warmth and professionalism.
  • Contribute to care planning discussions and participate in interdisciplinary case reviews where clinically relevant.
Program development
  • Play an active role in the development of Stem Health's Mental Health Program contributing clinical insights, therapeutic frameworks, and evidence-based protocols that shape how mental health is delivered across the organization.
  • Help develop member-facing mental health resources, psychoeducation materials, and wellness content that reflect Stem Health's standards of quality, evidence, and member-centred design.
  • Contribute to the ongoing refinement of the in-clinic mental health experience, identifying improvements to workflows, intake processes, session structure, and member communications.
Documentation and professional standards
  • Maintain accurate, timely, and thorough clinical documentation for all assessments, sessions, and care coordination activities within Stem Health's EHR platform, in compliance with OCSWSSW standards and PHIPA requirements.
  • Maintain registration in good standing with the OCSWSSW and adhere to all applicable standards of practice, ethical guidelines, and continuing education requirements.
  • Maintain current professional liability insurance required.

  • Registered Social Worker (RSW) in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) — required.
  • Minimum 3 years of post-registration clinical experience in direct therapeutic practice.
  • Demonstrated proficiency in evidence-based therapeutic modalities, including CBT, ACT, mindfulness-based interventions, or psychosocial counselling and navigation.

  • Proficiency with EHR/EMR systems and comfort working within a digitally integrated, multidisciplinary clinical environment.
  • Current professional liability insurance required.
Ideal Candidate

You are a clinician who brings both therapeutic precision and genuine human warmth to every session. You understand that proactive mental health care the kind that prevents crises rather than responding to them requires a different rhythm, a different level of trust, and a deeper commitment to relationships. You are energized by the opportunity to practice with the depth and time that your clinical training actually calls for, in a setting designed to support it. You want to be part of a team, not a solo practitioner in isolation and you are excited about helping build something new in a beautifully designed, hospitality-forward clinical environment.

Why Join Stem Health
  • A time-rich, low-volume model designed to protect the quality of therapeutic work trade caseload burnout for intentional, relationship-based practice in a beautifully designed clinical space.
  • True integration: you are a full member of a multidisciplinary clinical team at First Canadian Place, with access to a Unified Health Story that keeps everyone aligned and working together.
  • An active role in building Stem Health's Mental Health Program from the ground up your clinical perspective will shape the protocols, frameworks, and member experience.
  • A premium in-clinic environment designed to match the standard of care you deliver private, thoughtfully designed therapy spaces that create safety and presence from the moment a member arrives.
  • Meaningful growth: help define what proactive, in-person integrated mental health care looks like as a core pillar of private primary care in Canada.
