Position Overview:
Shkagamik-Kwe Health Centre is seeking a dedicated and compassionate Social Worker to join our team on a full-time, permanent basis. The successful candidate will play a pivotal role in delivering client-centred, culturally safe, and holistic social services. This position is designed for a professional who is committed to fostering community well-being, particularly within diverse populations, including Indigenous communities.
Key Responsibilities:
- Conduct comprehensive assessments of clients’ social, emotional, and physical needs.
- Develop and implement individualized care plans in collaboration with clients, families, and multidisciplinary team members.
- Provide ongoing case management, crisis intervention, and follow-up services.
- Offer therapeutic interventions including counselling and support groups tailored to clients facing mental health challenges, substance abuse issues, family conflicts, or other social concerns.
- Facilitate empowerment and resilience-building strategies for clients.
- Liaise with community resources, healthcare providers, and government agencies to secure additional support and services for clients.
- Advocate for client rights and access to social services and community programs.
- Provide culturally sensitive care and maintain an understanding of Indigenous cultural practices, values, and protocols.
- Collaborate with team members, including healthcare professionals and community leaders, to integrate culturally relevant support into care plans.
- Maintain accurate and confidential client records in accordance with privacy legislation and organizational policies.
- Participate in regular team meetings, case conferences, and ongoing professional development.
- Contribute to the design and evaluation of programs and services that meet the evolving needs of our community.
- Provide feedback to improve service delivery and client outcomes.
Qualifications
- Bachelor/Master of Social Work (BSW/MSW) from an accredited institution is required.
- Registration with the Ontario Association of Social Workers (OASW) or equivalent professional designation.
- A minimum of 2 years’ experience in social work practice, preferably in a community health or public service setting.
- Experience working with diverse populations, particularly Indigenous communities, is an asset.
Skills & Competencies:
- Excellent interpersonal, communication, and crisis intervention skills.
- Strong understanding of case management, community resources, and advocacy.
- Ability to work both independently and collaboratively within a multidisciplinary team.
- High level of cultural competency and sensitivity, with a commitment to practicing in a culturally safe environment.
- Proficiency in record-keeping and documentation in compliance with confidentiality standards.

- Flexibility to participate in outreach activities and community events.
- Ability to manage a varied caseload and work under deadlines.
Working Conditions:
- This position is based at the Shkagamik-Kwe Health Centre in Sudbury, Ontario.
- The role may require occasional travel for community outreach and professional development.
- Standard working hours with potential on-call responsibilities as needed to support client care.
