Job summary

The Hamilton Family Health Team is seeking two full-time Registered Practical Nurses to join the Lincoln Medical Centre in Beamsville, ON. You will support physicians and the interprofessional team by providing nursing care, performing clinical procedures, and coordinating follow-up to help patients navigate their care.

The ideal candidate is organized, stays calm under pressure, and is committed to respectful, culturally safe, trauma-informed care within a primary care clinic.

Qualifications

  • Current registration in good standing with the College of Nurses of Ontario (CNO) as a Registered Practical Nurse (RPN).
  • Current CPR (BLS) certification.
  • Demonstrated competence in primary care nursing skills (e.g., immunizations, injections, wound care, phlebotomy/specimen collection, PAP/HPV screening, point-of-care testing) or willingness to be trained.
  • Experience using an electronic medical record (EMR) and strong documentation practices.
  • Knowledge of infection prevention and control (IPAC) standards and routine practices.
  • Strong interpersonal communication, time management, and ability to prioritize in a busy clinic setting.
  • Ability to work independently and collaboratively within an interprofessional team.
  • Clear vulnerable sector check (or ability to obtain, as required by clinic policy).
  • 1–3 years’ experience in primary care, family practice, community health, or an ambulatory clinic.
  • Experience supporting chronic disease programs (e.g., diabetes education, spirometry support, anticoagulation support—if within clinic services) preferred.
  • Familiarity with Ontario health system navigation, community resources, and referral pathways preferred.
  • Additional certification/training (e.g., foot care, smoking cessation, wound care, phlebotomy) preferred.

Responsibilities

  • Perform nursing assessments (e.g., vitals, focused assessments) and document findings accurately in the EMR.
  • Provide in-clinic general nursing care for scheduled and walk-in visits as appropriate (rooming patients, medication reconciliation, history updates).
  • Work in collaboration with physicians to complete well baby visits, prenatal and annual physical assessments (including HPV screening)
  • Assist with minor procedures (e.g., wound care, dressings, suture/staple removal, ear lavage, wart removal) and maintain procedure trays/supplies.
  • Preparing equipment and ensuring that examination rooms are clean and stocked with required supplies and materials.
  • Triage patient calls, supporting patient callbacks related to lab requests and results, test results and prescription requests using established protocols; escalating concerns as required.
  • Maintain vaccine cold chain, IPAC practices, and clinic safety standards.
  • Contribute to quality improvement initiatives
  • Support a respectful environment by collaborating with colleagues and communicating professionally with patients and families.
  • Performing other related duties within the clinic as required.
