Job summary

Kingston Health Sciences Centre (KHSC) at Amherstview site is seeking a part-time Registered Practical Nurse to provide compassionate care in the Transitional Care Unit. You will assess, plan, and deliver patient-centered nursing care within the RPN scope.

The role offers an opportunity to work with an interdisciplinary team, gain experience in rehabilitation, geriatrics, and palliative care, and participate in discharge planning and care coordination while maintaining a strong patient-family

Qualifications

• Diploma from an RPN program and active registration in good standing with the CNO (or eligible).
• Knowledge of CNO standards and RPN scope of practice essential.
• Proven ability to work in a fast-paced healthcare environment and attend work regularly.

Responsibilities

• Assess, plan, implement and evaluate patient care within RPN scope.
• Deliver evidence-informed nursing care aligned to patient goals.
• Monitor progress, identify changes, and coordinate with the care team.
• Develop and coordinate individualized care plans.
• Support patients and families through rehabilitation and palliative care journeys.
• Delegate, teach, and supervise unregulated care providers.
• Participate in discharge planning and care coordination.
• Collaborate with physicians and allied health professionals to ensure seamless care.
• Contribute to quality improvement and foster a positive practice environment.
• Mentor students and team members.

About the Transitional Care Unit (TCU)

The Transitional Care Unit (TCU), located at the Alternative Healthcare Facility in Amherstview, is a 120-bed program supporting patients who require ongoing care while awaiting their next stage of care. Patients may be:

• Working toward increased independence through rehabilitation
• Receiving palliative and comfort-focused care

Our Nurse Practitioner-led model promotes a collaborative approach that allows team members to build meaningful relationships with patients and contribute to their ongoing care journey.

What You'll Do

As a Registered Practical Nurse, you will:

• Assess, plan, implement, and evaluate patient care within the RPN scope of practice

Required Qualifications
