Job summary

Baycrest Hospital in Toronto is seeking a Registered Practical Nurse (RPN) for the Complex Continuing Care Unit (5W) on a temporary part-time basis. The position offers Day shifts with alternate weekends and a 30-hour bi-weekly schedule.

You will plan, deliver and evaluate nursing interventions within the RPN scope, collaborate with the interdisciplinary team and help develop individualized care plans for medically complex older adults.

Qualifications

• Current registration, in good standing, from the College of Nurses of Ontario (CNO) is required.
• Current Basic Cardiac Life Support (BCLS) Level C with AED is required.
• Experience in geriatrics with complex medical care is preferred.
• Psychogeriatric experience, i.e. mental health assessment and intervention and working with responsive behaviours preferred.

Responsibilities

• Plans for, delivers and evaluates nursing interventions, including medications and treatments within the RPN scope of practice.
• Provides client-centred care within the interdisciplinary care team to meet personal care, activation, recreation and spiritual needs of clients.
• Collaborates with the Registered Nurse to develop and revise the individualized care plan.

Job Summary

The Complex Continuing Care Unit provides care for older patients with complex medical conditions in a non-acute hospital setting. The Complex Continuing Care Program helps patients and their families identify strategies that support quality-of-life goals as well as short term and long term care needs.

Key Responsibilities

• Plans for, delivers and evaluates nursing interventions, including medications, as well as treatments which fall within the RPN’s scope of practice
• Provides client-centred care within interdisciplinary care team to meet the personal care, activation, recreation and spiritual needs of clients
• Collaborates with the Registered Nurse to develop and revise the individualized care plan
