ParaMed Home Health Care in Sturgeon Falls, Ontario, is seeking a Registered Practical Nurse (RPN) to perform scheduled community visits and provide high-quality nursing care in patients’ homes. You will develop and monitor care plans, document patient encounters, and work with RNs for more complex cases.

You will travel within a designated area, maintain professional records, and participate in after-hours on-call coverage to ensure patient safety and continuity of care.

Qualifications

• Registration with the CNO as a Registered Practical Nurse in good standing.
• Valid driver’s license and access to a vehicle is required.
• Experience delivering community care is an asset.
• Reliability and flexibility are essential.
• Willingness to travel to patient residences within a designated area.

Responsabilités

• Participate in scheduled visits in the community to look after patient needs within scope of practice.
• Provide ethical and safe nursing care.
• Assess patients’ needs using appropriate tools and adjust care plans as required.
• Engage patients with a collaborative approach, building rapport and trust.
• Maintain and update patient records related to encounters and plans.
