Job Description

You will be responsible for providing high-quality nursing care to patients in the community. You should have clinical skills and knowledge of wound management, infusion therapy, palliative care, and chronic disease management.

  • Provide patient-focused, safe, holistic nursing care and work collaboratively with other members of the health care team.
  • Ensure that nursing care provided to our patients is based on best practices to achieve successful outcomes.
  • Continuously assess, anticipate, identify, and respond to the changing needs of our patients.
  • Travel within your region between patients’ homes.
Qualifications
  • Minimum two years of nursing experience (community nursing experience is ideal).
  • Registered and in good standing with the College of Nurses of Ontario (CNO).
  • Ability to work independently in the community.
  • Proficiency in English, both verbal and written, required (additional languages are an asset).
  • Valid Ontario driver’s license and reliable vehicle.
  • Experience with infusion therapy, mental health, palliative care, and wound care.
  • Excellent patient assessment and teaching skills.
