Responsibilities and Duties
  • Reviews the care plan arranged by the case manager prior to beginning a visit, complete records and carrying all assignments as requested.
  • Provide meal preparation, light housekeeping, and/or provide companionship and personal care, assists clients in all activities of daily living/hygiene assistance, bathing, grooming, linen changes, assist with transfers, ambulation and exercises and so forth.
  • Perform all skills, treatments and procedures competently in accordance to the provincial scope of RPN and adheres to the policies, procedures and protocols of Nurse Next Door.
  • Assess and provide client and family/caregiver education and information pertinent to diagnosis.
  • Nursing tasks include medication administration, wound care, head to toe assessment, pain assessment, liaising with physicians, diabetic management.
  • Excellent customer service skills and ability to deal tactfully with clients, other interdisciplinary team members.
  • Responds to Care Services Center (CSC) Team regarding scheduling inquiries and updates within the agreed upon timeline.
  • Engages in open communication with your Care Designer when opportunities to elevate your client’s experience and care are identified.
  • Meet the safety needs of the client and use equipment safely and properly.
  • Service the Brantford, Paris and nearby areas.
Requirements
  • Previous nursing experience is an asset.
  • Ability to identify and report client care concerns to Care Designers when needed.
  • Valid CNO license.
  • Must be able to communicate verbally and in writing in English; other languages are an asset.
  • Background or Criminal Record Check with a Vulnerable Sector Search (updated every 5 years for continued employment).
  • Negative TB Screen (updated every 5 years for continued employment).
  • Covid – 19 Vaccination Confirmation or medical exemption letter.
  • Valid driver’s license is required.
