A leading community support service in Ontario seeks a Registered Practical Nurse for their Home Programs. The role involves providing nursing care to clients and facilitating effective care transitions, emphasizing teamwork and proactive health management. Reporting to the Manager of Allied Health Services, the Registered Practical Nurse at Home Programs is responsible for providing and managing quality nursing care to clients transitioning out of hospital as part of Carefor’s atHome bundled care programs. The nurse manages wrap-around care coordinated and delivered to clients admitted to the program, with support from the care team. Responsibilities include completing InterRAI-HC assessments, developing care plans, engaging with team members (e.g., social workers, PTs, OTs, PTA/OTA, nutritional specialists, speech language pathologists), and delegating tasks to personal support workers.
The nurse utilizes assessment findings to proactively develop care plans that support interventions aimed at minimizing unnecessary emergency department visits and maintaining clients in their homes with the necessary support.
Additional responsibilities include providing clinical support, addressing PSW job performance issues, participating in client case conferences, offering ongoing training and assessments, and providing performance feedback. The nurse supports the care team and clients in delivering safe, client-centered care by modeling standards, defining care processes, and collaborating on PSW education and training. The position also involves supporting the implementation of evidence-based practice projects and initiatives within the atHome Program.
