Job summary

University Health Network (UHN) is seeking a Registered Practical Nurse for a replacement role at West Park Healthcare Centre in Toronto. You will provide compassionate, collaborative care within a multidisciplinary team, focusing on neuroscience and geriatric patients, aligned with professional practice standards.

The role requires current CNON registration, a Practical Nursing diploma, and experience with the elderly population; NIH certification is an asset.

Qualifications

• Valid registration with the College of Nurses of Ontario.
• Diploma in Practical Nursing.
• Recent related clinical experience in neuroscience nursing preferred.
• Neuroscience Nursing Certification an asset.
• Experience working with Geriatric and Frail Seniors population.
• Effective problem recognition, problem solving and decision making skills.

• Resilience and effective coping strategies to manage change.
• Demonstrated computer literacy in word processing and Electronic Patient Record/HIS systems.

Responsibilities

• Plan, manage, and evaluate nursing care for patients and families within the RPN scope of practice in the context of an interdisciplinary practice.
• Use current theoretical and research knowledge and the input of team members to plan and implement care for patients and families.
• Participate in inter-professional team care planning forums, inter-professional rounds and family conferences.
• Provide mentorship to RPN students and new staff on the unit.
• Participate in professional, program and hospital-wide initiatives, including professional development of self.
• Compliance with and commitment to patient safety responsibilities and corresponding hospital plans, policies and procedures in order to ensure a safe environment for patients.

Reports to: Manager, Program Services

Wage Range: $38.42 - $39.02 per hour (start rate in accordance with collective agreement requirements)

Hours: 37.5 hours per week

Shifts: Days/Evenings

Status: Permanent Full-Time

Position Summary

As a member of a multi-disciplinary Team, the Registered Practical Nurse practices in accordance with relevant legislation/standards, the UHN Professional Practice Model and the mission and vision of nursing at the UHN to provide compassionate, collaborative and competent care, while meeting the legal and ethical requirements of the profession.
