Job summary

University Health Network (UHN) is seeking a Registered Practical Nurse to join the Geriatric Functional Enhancement Service (2D) at West Park Healthcare Centre in Toronto. You will plan, manage and evaluate nursing care for patients within the RPN scope and work with an interdisciplinary team to ensure safe, effective care.

The role requires a valid Ontario RPN registration, a Diploma in Practical Nursing, and a background in geriatrics; neuroscience experience and certification are assets.

Qualifications

• Valid registration with the College of Nurses of Ontario.
• Diploma in Practical Nursing.
• Recent related clinical experience in neuroscience nursing preferred.
• Neuroscience Nursing Certification an asset.
• Experience working with geriatric and frail seniors population.
• Effective problem recognition, problem solving and decision making skills.
• Demonstrated commitment to continuing professional development.

• Resilience and effective coping strategies to manage change.

Position Summary

As a member of a multi-disciplinary Team, the Registered Practical Nurse practices in accordance with relevant legislation/standards, the UHN Professional Practice Model and the mission and vision of nursing at the UHN to provide compassionate, collaborative and competent care, while meeting the legal and ethical requirements of the profession.

Duties

• Plan, manage, and evaluate nursing care for patients and families within the RPN scope of practice in the context of an interdisciplinary practice.
