To use nursing process in providing patient care. To participate as a member of the Health Care Team and the Nursing Team. As an individual practitioner, to maintain competence and professionalism relevant to current practice. To maintain nursing records. Function as a member of a Multidisciplinary team. To train patients, family members, community agency staff and others. To perform home hemodialysis in the home environment; to manage all aspects of care for the dialysis patient; to provide patient and family with advice, counselling and teaching related to the condition. Must be able to travel to complete home visits for patients. Travel between Lakeridge Health sites as required. to perform other related duties as assigned.
