Job summary

SE Health is hiring a Registered Practical Nurse for Journey Home Hospice in Toronto. Earn a $2000 sign‑on bonus and join a team that supports your goals with strong total rewards and benefits.

The RPN will provide core hospice nursing care within a trauma-informed, culturally safe program for vulnerable populations, in a 13-bed downtown Toronto setting with opportunities for growth.

Qualifications

• Current Certificate of Competence with the College of Nurses of Ontario as a Registered Practical Nurse (RPN).
• LEAP Certificate required or willing to complete within 6 months of hire.
• Experience in Hospice Palliative care and with vulnerable populations is preferred.
• First Aid/CPR certification is required.

Responsibilities

• Provide compassionate hospice palliative care to patients in a home-like setting.
• Collaborate with an integrated health-care team to deliver full-scope nursing care.
• Maintain accurate clinical documentation and adhere to quality improvement processes.

Requirements:

• Current Certificate of Competence with the College of Nurses of Ontario as a Registered Practical Nurse (RPN)
• LEAP Certificate required / Willing to complete LEAP Certificate within 6 months of hire
• Minimum 1 year Hospice Palliative care experience and ongoing continuing education is preferred
• Experience working with vulnerable populations
• Mental Health Nursing Certificationor equivalent training preferred
• 1 year Mental Health experience and ongoing continuing education is an asset
• Well-developed interpersonal skills and excellent organizational skills
• Able to work independently as well as part of an integrated health care team (PSWs, RPNs, Physician, Nurses, Hospices Volunteers)
• Effective problem-solving and conflict management skills
• Works well in a changing environment with an appreciation for continuous quality improvement processes
• A current First Aid and CPR certificate is required
• Availability to work 12 hour shifts
