Job summary

ParaMed Home Health Care is seeking a Registered Practical Nurse to perform scheduled community visits and provide patient-centered nursing care in home settings with RNs available for support on complex cases. This role offers autonomy, independence, and a balanced work-life arrangement within a supportive nursing team.

In Ontario, you’ll work with patients of various ages, maintaining comfortable home environments.

Qualifications

  • Registered Practical Nurse (RPN) must be in good standing with CNO.
  • Valid driver’s license and access to a vehicle.
  • Experience delivering community care is an asset.
  • Reliability and flexibility to travel within designated area.
  • Willingness to provide on-call coverage, including after hours.

Responsibilities

  • Participate in scheduled community visits to assess and address patient needs within scope of practice.
  • Provide ethical and safe nursing care.
  • Use assessment tools to evaluate patients’ conditions and adjust care plans.
  • Execute and monitor care plans; adjust as required.
  • Engage patients with a collaborative approach and build rapport and trust.
  • Maintain accurate patient records and encounter notes.

In this community nursing role, you as a Registered Practical Nurse (RPN) will have scheduled visits within the community to care for patients in various settings with the support of RNs for more complex patients. You will work with patients of all ages with differing complexity of care needs and play a key role in helping patients remain comfortable in their homes for as long as possible. What You’ll Do
  • Participate in scheduled visits in the community to look after patient needs within your scope of practice
  • Provide ethical and safe nursing care
  • Use appropriate assessment tools/techniques to assess patients' individual needs
  • Execute and continuously monitor care plans and adjust as required
  • Engage patients using a collaborative approach by building rapport and trust
  • Maintain and update professional patient records related to patient encounters and plans

  • Registration with the CNO as a Registered Practical Nurse in good standing
