Job description

Your Opportunity:

This Transition Coordinator position is clinical consult-based and located at the Royal Alexandra Hospital, where you will be part of a team of Transition Coordinators managing an assigned portfolio/inpatient unit(s), including Emergency. As a TC, you will assess patients’ needs considering their overall situation, including social determinants of health. Critical thinking and accurate interpretation of complex information from various sources are essential for clinical decision-making regarding appropriate continuing care options. The role involves comprehensive assessment, planning, coordination of care, and ongoing communication with healthcare providers across the continuum of care. Upon hire, you will receive specialized training to equip you with the necessary knowledge and tools. Major responsibilities include conducting electronic needs assessments, prioritizing urgency, developing service plans, coordinating service delivery, liaising with Continuing Care and community resources, and providing support to clients, families, and caregivers. The TC works within a multidisciplinary team and is responsible for referrals, documentation, participating in meetings, evaluating processes and outcomes, and engaging in professional development activities. Applicants should be aware of physical demands related to patient assessment and coordination.

Description:

As a Registered Nurse (RN), you will provide diverse nursing services to patients, families, communities, and populations, ensuring safety and well-being. You will utilize nursing processes, employing critical thinking, problem-solving, and decision-making, while also teaching, counseling, and advocating for patients and families. You will support an integrated, holistic approach to patient care, health promotion, and maintenance, providing safe, quality, patient- and family-centered care aligned with AHS values.

Note: Provisional Permit Holders will be paid $35.87/hour - $43.82/hour. Once registered as a RN, pay will be adjusted according to applicable AHS/UNA agreements. This posting closes at 23:59 MT on the end date but may be filled earlier if a current employee expresses interest by 23:59 MT on the fifth day after posting. Required Qualifications:

  • Completion of an accredited nursing program
  • Active or eligible registration with the College of Registered Nurses of Alberta (CRNA)
  • Current Basic Cardiac Life Support - Healthcare Provider (BCLS-HCP)
  • For those not practicing in the last five years, completion of CRNA Medication Management and NextGenU Health Assessment courses is required

Additional Required Qualifications:

  • At least five years of facility-based nursing experience, including geriatric care within the last three years
  • At least two years of community nursing experience
  • Strong time management, communication, conflict resolution, and problem-solving skills
  • Ability to work independently and in teams, with capacity to handle stress and physical demands

Preferred Qualifications:

  • Knowledge of relevant computer applications such as Microsoft Word, Pathways, RAI Tools, and e-People, Connect Care
  • Active Transition Coordinators with current certification may be preferred
