Edmonton Zone Continuing Care provides services through five main program areas: Home Living, Supportive Living, Facility Living, Transition Services, and Palliative Care. We are looking for a Registered Nurse to join our Home Living team to provide professional support services in the homes of clients of all ages and stages of life.
As a self-directed team member within Continuing Care in the Edmonton zone, the Registered Nurse collaborates with the healthcare team and is responsible for assessment of needs, delivering therapeutic nursing services within professional standards, coordinating resources, establishing client goals, and evaluating outcomes. This role involves case management and direct client care that adheres to clinical standards, evidence-based practices, and client-centered approaches. The RN acts as a clinical expert and team consultant, develops care plans based on unmet needs, and collaborates with family and partners. Additionally, the RN participates in staff development, mentoring students, and facilitates complex discharges from Acute Care to Home, working with Transition Coordinators and multidisciplinary teams to support client flow to Home Living options.
Required: Completion of an accredited nursing program, active or eligible registration with the College of Registered Nurses of Alberta, current BCLS-HCP certification, and recent nursing experience (within last 5 years) or completion of a refresher program. Additional courses may be required if not practicing in the last five years.
Additional: Self-directed, independent work ability, excellent organizational, communication, and assessment skills, computer proficiency, physical capability for nursing tasks and working in various environments.
Baccalaureate degree in nursing preferred; palliative care, geriatric, or Continuing Care experience, familiarity with Connect Care and RAI tools, and case management experience are advantageous.
