Job Summary:

The Registered Nurse is responsible for providing comprehensive, evidence-based nursing care to clients and families in their homes and community settings. This includes assessments, symptom management, palliative and rehabilitative care, wound care, education, and counseling, ensuring care aligns with quality standards, risk management, and scope of practice set by VON, provincial regulatory bodies, and the Community Health Nurses of Canada. The RN also oversees the delegation and supervision of tasks to various care team members.

Key Responsibilities:

  1. Manage and coordinate care, evaluate health outcomes, and advocate for clients to meet health goals.
  2. Deliver all levels of nursing care, from stable to complex, acute cases.
  3. Provide cost-effective, outcome-focused care, including delegation of tasks.
  4. Ensure care delivery complies with established standards and policies.
  5. Conduct comprehensive assessments of clients, families, and environments.
  6. Develop and implement individualized care plans, incorporating discharge planning as needed.
  7. Perform nursing interventions according to care plans, documenting processes accurately.
  8. Utilize effective communication skills to gather information and evaluate responses.
  9. Promote client, family, and caregiver involvement in care planning and decision-making.
  10. Support health and well-being through education and health promotion strategies.
  11. Collaborate with the healthcare team, referring to other providers as appropriate.
  12. Contribute to policy development and practice standards through professional involvement.
  13. Maintain compliance with quality, risk management, and occupational health and safety policies.
  14. Participate in quality improvement initiatives and self-evaluation of practice.

Education, Designations, and Experience:

  • Licensed as a Registered Nurse in the practice province.
  • Canadian Nurses Association certification in Community Health Nursing is an asset.
  • Additional degrees or certifications may be required for specialized roles.
  • Clinical practice experience and community health experience are assets.

Working Conditions and Physical Demands:

This role involves a detailed-oriented approach in a dynamic environment, with physical activities including lifting, carrying, bending, reaching, and other movements emphasizing good body mechanics. Tasks include walking, sitting, standing, climbing stairs, and fine motor activities.
