Job Summary:
The Registered Nurse is responsible for providing and managing client and family-centered, evidence-based, efficient nursing care to individuals and families in their homes and community. The RN ensures care is delivered within quality & risk management frameworks and adheres to standards set by VON, provincial regulatory bodies, and the Community Health Nurses of Canada. The RN also oversees the safe teaching, assignment, and delegation of tasks to various categories of workers.
Key Responsibilities:
- Manage and coordinate care; evaluate health outcomes; educate, counsel, and advocate for individuals to meet health goals.
- Provide all levels of nursing care, whether for stable clients with predictable outcomes or for those with acute, complex, and rapidly changing needs.
- Deliver cost-effective, organized, outcome-oriented care, and delegate as necessary.
- Ensure service delivery aligns with VON standards, provincial regulations, and national guidelines.
- Conduct comprehensive assessments of clients, families, and caregivers in their environment.
- Develop and implement care plans collaboratively with clients and families, incorporating discharge planning as needed.
- Implement nursing interventions according to care plans.
- Perform technical nursing skills safely, following nursing principles and policies.
- Document nursing processes, client responses, and care plans accurately and clearly.
- Use effective communication skills to gather information and evaluate responses to care plans.
- Promote client, family, and caregiver involvement in care decisions.
- Support health and well-being through education and engagement to foster independence.
- Negotiate care delivery with clients and families respecting their choices.
- Incorporate current research into clinical practice.
- Collaborate with the healthcare team, report regularly on client status, and refer to appropriate providers and disciplines.
- Manage resources effectively and serve as a clinical resource and mentor.
- Promote client-directed care and health promotion through teaching.
- Contribute to policy review and development within healthcare and nursing standards.
- Practice within VON's quality and risk management frameworks and occupational health and safety legislation.
- Participate in ongoing quality improvement and maintain practice competence.
- Perform other duties as assigned.

Education, Designations, and Experience:
- Licensed as a Registered Nurse in the practicing province.
- Canadian Nurses Association certification in Community Health Nursing is an asset.
- Additional degrees or certificates may be required for specialized roles.
- Clinical practice and community health experience are assets.
