Job Summary:
The Registered Nurse with advanced and specialized knowledge and clinical skills in wound, ostomy, and continence care who has graduated from a recognized education program is responsible for providing and managing client and family-centered, evidence-based, efficient nursing care to individuals and families in their homes and communities. The RN ensures care is provided within the framework of quality & risk management and the standards and scope of practice set by VON and relevant provincial regulatory bodies.
Key Responsibilities:
- Performs detailed holistic assessments of clients with wound, ostomy, and/or continence concerns.
- Develops treatment plans in collaboration with the interprofessional team based on holistic assessments.
- Provides education and mentorship to build wound, ostomy, and continence care capacity within VON.
- Offers consultation to care providers within the organization.
- Collaborates across the continuum of care with the interprofessional team.
- Shares clinical knowledge to enable informed health choices by clients, families, and communities.
- Stays updated on current evidence-based practices related to wound, ostomy, and continence care.
- Provides services in accordance with VON standards and provincial regulations.
- Documents nursing processes clearly, accurately, and according to standards.
- Records client and family responses to care interventions.
- Engages clients and families in health and service planning.
- Negotiates care delivery to respect client choices.
- Documents referrals, visit adjustments, and discharges based on sound data and guidelines.
- Refers clients to appropriate nursing and other health providers.
- Utilizes tools and resources efficiently and manages these resources effectively.
- Promotes client self-care, independence, and health education using adult learning principles.
- Contributes to policy review and development through involvement in professional organizations.
- Practices within the quality and risk management framework and health and safety legislation.
- Participates in creating and following up on client care plans to support quality improvement.
- Performs other duties as assigned.

Education, Designations, and Experience:
- Must be licensed as a Registered Nurse in the province of practice.
- Completion of NSWOCC Wound, Ostomy & Continence Canada (WOC-EP) program, or IIWCC, or be a Master’s prepared APN with formal skin and wound care education.
- Three years of wound care experience in community or acute care settings preferred.
- Experience in community and home care is preferred.

Working Conditions and Physical Demands:
This role requires attention to detail in a dynamic environment, involving physical activities such as lifting, carrying (with proper techniques), bending, reaching, kneeling, and other movements emphasizing good body mechanics. Tasks include walking, sitting, standing, climbing stairs, and fine hand movements throughout the day.
