Job Summary:

Working under the direction of the Manager, Utilization, Access and Flow, the RN Utilization Coordinator’s (‘RNUC’) primary role is to conduct clinical assessments of patients seeking admission to Baycrest to ensure appropriate intake decision making into Baycrest programs. The role will focus on building relationships with referral partners and easing the transition for complex clients being referred to Baycrest services and programs.

Responsibilities include but are not limited to:

  • Provides expertise in capacity/bed management and patient flow, and in understanding and supporting the clinical implications of admission, discharge, transfer and repatriation of older adults experiencing physical, functional and cognitive change and/or complex medical issues.
  • Identifies, initiates and facilitates the intake and admission process for patients who meet established admission criteria and would benefit from Baycrest programs, ensuring timely decision-making and response.
  • Provides on-site clinical assessments of candidates as appropriate using comprehensive nursing assessments, interviews, standardized and observational assessment techniques.
  • As a key member of the program team, involves, consults and communicates with team members as appropriate during the application process.
  • Proactively coordinates a streamlined process for transitioning patients through the admission and transfer process, working closely with the referring partners, to optimize flow and resource utilization.
  • Establishes strong relationships with internal and external stakeholders to facilitate communication, utilization and information-sharing to remove barriers to patient flow.
  • Participates in process improvement activities with internal and external stakeholders including interprofessional teams, ADT, decision support, acute care and other referral sources, and internal/external task-groups.
  • Proactively participates in process and improvement activities using available data and information, with internal and external stakeholders including inter-professional teams, ADT, decision support, acute care and other community partners.

Qualifications include but are not limited to:

  • Current registration, in good standing, from the College of Nurses of Ontario (‘CNO’) is required.
  • Current Basic Cardiac Life Support (‘BCLS’) certificate, Level C - Automated External Device (‘AED’) is required.
  • BScN required, Master’s degree preferred.
  • Canadian Nursing Association certificate in Rehab or Gerontology and/or Palliative Care preferred.
  • Minimum 3-5 years’ relevant clinical Rehab, Geriatrics, Complex Continuing Care and/or Palliative Care experience.
  • Strong knowledge of the physical, functional and cognitive complexities of the older adult and ability to assess rehab potential, medical complexity and end of life care needs.
  • Knowledge and familiarity with resource management and referral (RM&R) community resources, relevant healthcare legislation and utilization pressures.
  • Familiarity with Ontario Health Teams is an asset.
  • Ability to travel to attend offsite meetings and in-person clinical assessments as appropriate and required.
  • Excellent interpersonal, communication and problem solving skills.
  • Basic computer skills – Meditech, Microsoft Word Outlook, and Excel and use of RM&R, MDS, NRS preferred.
  • Knowledge of widely utilized clinical assessment tools (ie: NRS, FIM, MoCa, MDS, MMSE, GDS, ESAS, PPS) preferred.
