Job summary

Prosperity Workforce Solutions is recruiting a Licensed Practical Nurse, Long Term Care for a permanent position in Kelowna, BC. The role supports an inter-professional healthcare team, delivering person-centered care and comprehensive documentation.

The successful candidate will hold a Practical Nurse Diploma, active BCCNM registration, and current CPR/First Aid certifications, with experience in Canadian long-term care. Shift: full-time nights, 630pm-630am, on a rotating schedule.

Qualifications

• Practical Nurse Diploma required.
• Active BCCNM License Registration.
• Experience in Canadian senior/long-term care.
• CPR training certification.
• First Aid certification.
• Criminal Record and Vulnerable Sector Check completed or willingness to obtain.

Responsibilities

• Work as part of an inter-professional team delivering person-centered care.
• Complete and ensure resident care assessments, care/service plans, and documentation.
• Implement and evaluate care plans and update as needed.
• Assist with scheduling and participate in care conferences.
• Coordinate with Director of Care on move-ins, move-outs, transfers.
• Provide direct resident care and medication/treatment administration per standards.

Location: Kelowna, BC

Hours:

• Full time night line, 630pm-630am, rotating between 3 days on, 2 days off, 3 days on, 3 days off

Responsibilities:

• Work as a member of the inter-professional team as described by our Professional Practice Model applying the person-centered approach to care.
• Complete and ensure resident's clinical care assessments, care/service plans and documentation are accurate, up to date and changes communicated as required.
• Implement and evaluate the effectiveness of the care/service plans and make changes as required.
• Assist with scheduling of resident's care conferences and participate in conferences where applicable.
• Work closely with the Director of Care to facilitate resident's move in, move out and transfers.
• Provide direct resident care and medication/ treatment administration and document care in compliance with regulatory and Resident Care standards, policies, and procedures.
  

Qualifications:

• Practical Nurse Diploma
• Active BCCNM License Registration
• Prior Canadian experience in senior care or long-term care
• Current CPR training certification
• Current First Aid certification
• Completion of a current Criminal Record and Vulnerable Sector Check (or willingness to obtain)
