Job summary

Prosperity Workforce Solutions is recruiting a Licensed Practical Nurse, Long Term Care to join a Kelowna, BC team for a permanent full-time night shift role.

Location: Kelowna, BC. The position involves inter-professional collaboration, resident assessments, care planning, medication administration and documentation in accordance with regulatory standards.

Qualifications

• Practical Nurse Diploma required.
• Active BCCNM License Registration.
• Prior Canadian experience in senior or long-term care.
• Current CPR certification.
• Current First Aid certification.
• Criminal Record and Vulnerable Sector Check completed or willingness to obtain.

Responsibilities

• Work as part of an inter-professional team applying person-centred care.
• Complete and ensure resident care assessments and care/service plans are current.
• Implement and evaluate care/service plans and adjust as needed.
• Assist with scheduling and participate in resident care conferences.
• Collaborate with the Director of Care on moves, transfers and admissions.
• Provide direct resident care, medication/treatment administration and documentation in line with standards.

Hours:

• Full time night line, 630pm-630am, rotating between 3 days on, 2 days off, 3 days on, 3 days off

Responsibilities:

• Work as a member of the inter-professional team as described by our Professional Practice Model applying the person-centered approach to care.
• Complete and ensure resident's clinical care assessments, care/service plans and documentation are accurate, up to date and changes communicated as required.
• Implement and evaluate the effectiveness of the care/service plans and make changes as required.
• Assist with scheduling of resident's care conferences and participate in conferences where applicable.
• Work closely with the Director of Care to facilitate resident's move in, move out and transfers.
• Provide direct resident care and medication/ treatment administration and document care in compliance with regulatory and Resident Care standards, policies, and procedures.

Qualifications:

• Practical Nurse Diploma
• Active BCCNM License Registration
• Prior Canadian experiencein senior care or long-term care
• Current CPR training certification
• Current First Aid certification
• Completion of a current Criminal Record andVulnerable Sector Check (or willingness to obtain)
