Northern Health in Terrace, BC is seeking a Patient Care Coordinator (PCC) to lead day-to-day clinical operations and ensure high-quality patient care. The PCC will guide staff, serve as a clinical resource, and support patient and family education within a strong, patient-centred care model.

The role emphasizes collaboration with the interdisciplinary team, policy development, staffing planning, and ongoing quality improvement to enhance care delivery across designated areas.

Qualifications

• Graduation from an approved School of Nursing; advanced clinical specialty prep; 2 yrs recent clinical experience; 2 yrs supervisory/teaching exp or equivalent.
• Current practicing registration with BC College of Nurses and Midwives (BCCNM).
• Current CPR certification.

Responsibilities

Position Summary

In accordance with established vision and values of the organization, the Patient Care Coordinator (PCC) practices in accordance with the standards of professional practice and code of ethics as outlined by the BC College of Nurses and Midwives (BCCNM) as well as within a patient and family centered care model.

The PCC is responsible for the coordination of day to day operations and quality patient care within a designated clinical area(s). This position supports the seamless movement of patients through the healthcare system and in accessing appropriate resources. The PCC supervises as well as serves as a role model and a resource to staff for clinical decision-making, clinical assessments of patients and in the counselling and education of families. Reviews, analyses and makes recommendations on the development of administrative systems such as policies, procedures and standards of patient care, adequate staffing levels, and a continuous quality improvement environment. Liaises with the interdisciplinary team for identification, coordination and efficient utilization of resources as well as plans for staff educational needs.

Shift Rotation/Hours of work:

Days, Nights, 07:00 to 19:00, 19:00 to 07:00 (rotating)

Ksyen Regional Hospital

The new Ksyen Regional Hospital welcomed its first patients on November 24, 2024. More than double the size of the previous hospital, the facility features 82 beds, private rooms, and is a centre for trauma services, orthopedic surgeries, pathology, radiology, clinical support, and pharmacy services. The hospital is also a training site for medical students in the Northern Medical Program.

Qualfications

Graduation from an approved School of Nursing, advanced preparation in the clinical specialty of the assigned area, two years of recent related clinical experience in the assigned practice environment and two years recent supervisory and teaching experience or an equivalent combination of education, training and experience.

Current practicing registration with the BC College of Nurses and Midwives (BCCNM).

Current CPR certification required.
