Job summary

UHN is hiring a Registered Nurse for West Park Healthcare Centre in Toronto. The 20-hour-per-week replacement position focuses on geriatric rehabilitation and dementia care within an inter-professional team.

Successful candidates will hold a Nursing Diploma, may have a Baccalaureate in Nursing, and must be in good standing with the College of Nurses of Ontario. The role includes leadership, education, and ongoing professional development.

Qualifications

• Diploma in Nursing or equivalent required.
• Baccalaureate degree in Nursing preferred.
• Registered with the College of Nurses of Ontario in good standing.
• Ability to perform essential clinical duties and manage multiple admissions in EPR.

Responsibilities

• Plan, manage, evaluate and coordinate comprehensive nursing care.
• Care for patients with dementia within inter-professional teams.
• Educate patients, families and facilities about dementia care.
• Lead and mentor students and new staff on the unit.
• Support professional development and participate in hospital-wide initiatives.
• Ensure patient safety and adhere to hospital policies.

Position Summary

The role of the Registered Nurse is the promotion of health and the assessment of, the provision of care for and the treatment of health conditions by supportive, preventative, therapeutic, palliative and rehabilitative means in order to attain or maintain optimal function.

Duties

• Plan, manage, evaluate and coordinate comprehensive professional nursing care for patients and families within the context of an inter-professional practice and primary nursing model
• Use theoretical and empirical knowledge and the input of team members to plan and implement care for patients with dementia and their families
• Education of patients, family, referring facilities and other community providers regarding care of older adults with dementia
• Participate in inter-professional team care planning forums, inter-professional rounds and family conferences
• Provide leadership, supervision and mentorship to students and new staff on the unit related to Geriatric Rehabilitation nursing
• Participate in professional, program and hospital-wide initiatives, including professional development of self
• Compliance with and commitment to patient safety responsibilities and corresponding hospital plans, policies and procedures in order to ensure a safe environment for patients

• Diploma in Nursing
• Baccalaureate degree in Nursing from an approved nursing program preferred
• Member in good standing with the College of Nurses of Ontario
• Ability to perform the essential clinical duties of the job
