Job summary

Alberta Health Services is seeking a Registered Nurse for a designated float role covering planned and unplanned absences in Integrated Home Care. You will provide direct nursing care, develop client-centered care plans, and collaborate with families and an interdisciplinary team across three Calgary-area sites.

Requirements include CRNA registration eligibility, BCLS-HCP, and 5+ years of nursing with 2+ years in community/home care.

Qualifications

  • Completion of an accredited nursing education program.
  • Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA).
  • Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification.
  • 5+ years of nursing experience with a minimum of 2 years of community nursing experience including 1 year of recent Home Care nursing experience providing case management and nursing interventions.

Responsibilities

  • Provide nursing assessments, case coordination and direct care in clients' homes or alternate care settings.
  • Coordinate services with the Home Care team and interdisciplinary colleagues.
  • Collaborate with clients and families to identify needs and goals and develop care plans.
  • Report to the Home Care Manager and maintain documentation of care and interventions.

Job description

Your Opportunity:

This exciting opportunity, as Registered Nurse, Designated Float covers planned and unplanned staff absences. Integrated Home Care uses a strength–based Client Centered Case Management practice framework. Using a person-centered approach, you will develop a care plan supporting the client/family to identify needs, focus on strengths, set achievable personal health and self-care goals and provide direct nursing care. In collaboration with the client/family and interdisciplinary team you will ensure services are provided and modified on an ongoing basis, based on client’s response to interventions, incorporating stewardship into service authorization. You will report to the Home Care Manager and will be responsible for coordinating and providing quality care to clients in the community. You will provide nursing assessments, case coordination and direct care in partnership with short term and long-term clients and their families in their own residence or an alternate care setting, as determined by the assessment. The successful incumbent will be expected to demonstrate proficiency in the following areas: personal and professional accountability, systems thinking, creative thinking, communicating and building relationships, organizational and time management skills, interpersonal skills and intermediate level of computer skills. This role requires flexibility, adaptability and rural/remote travelling; possession of a valid drivers’ license and access to a motor vehicle in good repair for work related purposes. This position works out of 3 sites: Northwest Centre 1, East Calgary Health Centre and South Calgary Health Centre.

Description:

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values. PLEASE NOTE: Provisional Permit holders will be paid $40.76 to $55.79 per hour. Upon obtaining a full practice permit, the rate of pay will be adjusted to the applicable Registered Nurse rate in accordance with UNA Article 25.02(b)(i) or (ii), as applicable.

Required Qualifications:

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Additional Required Qualifications:

The candidate must have at least 5 years of nursing experience with a minimum of 2 years of community nursing experience including 1 year of recent Home Care nursing experience providing case management and nursing interventions. A valid Driver’s License and a vehicle in good repair. Ability to lift 30 lbs. You will be required to operate a laptop computer and mobile phone, and for manual handling (lifting, lowering, carrying, pushing and pulling) as a regular part of the job. Basic Cardiac Life Support – Health Care Provider (BCLS-HCP) from Heart and Stroke Foundation within the last 12 months. Level 2 McMaster AHS Case Management education an asset.

Preferred Qualifications:

Bachelor’s degree in Nursing; Experience working with acute clients/long term clients with a focus on chronic disease management; Experience providing case management, client teaching & professional nursing interventions including but not limited to Home Parenteral Therapy (HPT), wound care, ostomy care; Experience supporting vulnerable clients those requiring palliative, mental health/addictions support.
