Alberta Health Services seeks a Registered Nurse to join the Rural South Home Care team serving communities around High River and beyond. You will deliver direct nursing care, develop patient-centered care plans, and coordinate with families to adjust services as client needs evolve.

Your Opportunity: If you are interested in working within an inter-professional team playing a key role in linking the needs of Rural Home Care clients with appropriate health and community services, then this Registered Nurse position will be of interest to you. Based on the Continuing Care Case Management Competency Framework you demonstrate ability in the areas of personal and professional accountability, systems thinking, facilitating creative thinking, communicating and building relationships. Using a person-centered approach, you will develop a care plan supporting the client/family to identify needs, focus on strengths, set achievable personal health and self-care goals and provide direct nursing care to individuals across the life span. In collaboration with the client/family you will ensure services are provided and modified on an ongoing basis, based on client’s response to interventions, incorporating stewardship into service authorization. Please note that this position supports the communities 60km SE of the Calgary City Limits. The Rural South team is comprised of Diamond Valley, Okotoks, High River, Nanton, Claresholm and Vulcan. The farthest site is 160km south of the city.
As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values. PLEASE NOTE: Provisional Permit holders will be paid $40.76 to $55.79 per hour. Upon obtaining a full practice permit, the rate of pay will be adjusted to the applicable Registered Nurse rate in accordance with UNA Article 25.02(b)(i) or (ii), as applicable.
