Job summary

Extendicare is seeking a part-time Registered Nurse to lead and coordinate resident care in our Canadian long-term care facilities. You will supervise care delivery, manage medications, and ensure safe, compassionate outcomes for residents and families.

You will collaborate with the DOC/ADOC, supervise UCPs, and ensure adherence to infection control and quality improvement plans while supporting student placements and upholding Extendicare values.

Qualifications

  • Current certificate of competence from the provincial nursing regulator.
  • Registration or eligibility for registration with the provincial regulator.
  • Previous managerial experience in long-term care or community homecare preferred.
  • Experience with cognitively impaired residents an asset.
  • Comprehensive knowledge of nursing and health care practices, including infection control.
  • Commitment to working with seniors.

Responsibilities

  • Perform resident assessments and develop care plans in collaboration with residents and the interdisciplinary team.
  • Coordinate care conferences, admissions, transfers, and transitions with the Director of Care.
  • Oversee medication management and supervise self-medication auditing.
  • Supervise unregulated care providers and coordinate nursing assignments; act as supervisor in DOC/ADOC absence.
  • Educate residents and families and follow up on concerns.
  • Monitor resident conditions and coordinate care to achieve optimal outcomes.
  • Support IPAC programs, safety initiatives, and quality improvement efforts.
  • Coordinate nursing student placements and supervise their learning.
  • Maintain RN registration and uphold professional standards and Extendicare policies.

Job description

Job Responsibilities
  • Resident Assessment and Care Planning Complete and maintain resident assessments, care plans, and documentation. Ensure care plans are current and changes are communicated appropriately. Plan, coordinate, manage, and evaluate residents' care in collaboration with residents, substitute decision-makers, physicians, and the interdisciplinary team.
  • Care Conferences and Transitions Assist with scheduling resident care conferences and participate in them. Facilitate resident admissions, move‑ins, transfers, and transitions in collaboration with the Director of Care (DOC).
  • Medication Management Order, secure, and administer medications, including narcotics, in compliance with regulatory requirements and Extendicare policies. Supervise resident self‑medication assessments and audits.
  • Leadership and Supervision Supervise and delegate care and medication administration tasks to Unregulated Care Providers (UCPs) within their scope of practice. Coordinate nursing assignments to ensure timely and efficient resident care delivery. Act as supervisor and coordinate emergency responses in the absence of the DOC/ADOC.
  • Resident and Family Support Provide resident and family education. Respond to resident and family concerns and ensure appropriate follow‑up or escalation to leadership when required.
  • Clinical Management Respond to significant changes in resident condition and medical emergencies. Monitor, evaluate, and coordinate resident care to achieve optimal outcomes.
  • Quality, Safety, and Infection Prevention Assist with the coordination, implementation, and monitoring of: Infection Prevention and Control (IPAC) programs Health and Safety initiatives Clinical programs Nursing quality improvement plans.
  • Student Supervision Coordinate and supervise nursing student placements from approved educational institutions.
  • Professional Accountability Maintain RN registration and competencies required by the applicable nursing regulatory college. Uphold Extendicare's Code of Conduct, ethics, integrity, and professional standards.

  • Comprehensive knowledge of nursing and health care practices, as well as Knowledge in Infection Control Practices
  • Demonstrated commitment to working with seniors
  • Ability to present information clearly and effectively both verbally whether in person or over the phone, and in writing; an active listener who projects a friendly demeanour
  • Ability to collect and use information to thoroughly think through problems and situations; puts information from various sources together to see the ‘big picture’ and generate creative solutions
  • Organizational, planning, time management and multi-tasking skills
  • Flexibility to work in different roles as per the facility requirements
  • Willing to work on different units as per the facility needs
