Reporting to Manager of Continuing Care, the successful candidate will be responsible for assessing, planning implementation and evaluating nursing care and practice activities in a multidisciplinary environment. You will be providing care to a diverse patient caseload of post surgical, cardiac, stroke, musculoskeletal, progressive disorders, complex chronic illness, and dementia patients.
