Job summary

Credit Valley Family Health Team (CVFHT) is seeking aRegistered Nurse to provide comprehensive, patient-centred care at a satellite site. Organization Overview

The Credit Valley Family Health Team (CVFHT) is an academic Family Health Team affiliated with Trillium Health Partners and the University of Toronto. We are a collaborative, inter-professional primary care organization committed to continuous learning, quality improvement, innovation, and excellence in patient-centred care.

As a not-for-profit organization funded by the Ministry of Health, CVFHT provides comprehensive primary care and interdisciplinary health services to a diverse patient population, including a dedicated commitment to serving the Francophone community.

The successful candidate will be employed by CVFHT and will be primarily based at one of our satellite sites, working collaboratively with the physicians, healthcare providers, administrative staff, and community partners at that location while remaining fully integrated into the broader CVFHT team and programs.

Position Summary

The Registered Nurse (RN) provides comprehensive, evidence-informed, and patient-centred nursing care to patients accessing services at the satellite site.

Working within an inter-professional primary care environment, the RN is responsible for patient assessment, care planning, clinical interventions, health education, care coordination, follow-up, and evaluation of patient outcomes. The RN will work both independently and collaboratively, exercising sound clinical judgment and maintaining professional nursing standards in accordance with the College of Nurses of Ontario (CNO).

The RN will also contribute to the integration of the satellite site with the broader Family Health Team by participating in CVFHT programs, quality improvement initiatives, education, clinical projects, and organizational activities.

As the RN will be based at a satellite location, the successful candidate must demonstrate strong communication, organization, adaptability, and the ability to work independently while maintaining regular communication and collaboration with the CVFHT's clinical and operational teams.

Key Responsibilities

Clinical Care and Patient Support

• Conduct comprehensive and ongoing patient assessments to identify health needs, risks, and priorities
• Develop, implement, and evaluate individualized care plans in collaboration with patients and the care team
• Provide health education, counselling, disease prevention, and health promotion across the lifespan
• Deliver clinical interventions including (but not limited to): immunizations, injections, wound care, ear syringing, ECGs, spirometry, wart treatment, and ambulatory blood pressure monitoring
• Perform telephone triage and follow-up within scope of practice
• Support chronic disease management, well-baby care, and family health
• Maintain accurate, timely, and complete clinical documentation
• Promote patient safety, privacy, dignity, accessibility, and a positive patient experience

Satellite Site Operations and Integration

• Serve as an integral clinical member of CVFHT while working primarily from the designated satellite site
• Establish and maintain effective working relationships with physicians, clinicians, administrative staff, and other healthcare providers at the satellite location
• Collaborate with satellite site leadership and CVFHT Operations Manager to support effective day-to-day clinical operations
• Communicate regularly with the broader CVFHT clinical and operational teams to ensure consistency in processes, standards, and patient care
• Follow CVFHT policies, procedures, clinical protocols, documentation standards, privacy requirements, and health and safety practices
• Support the implementation of CVFHT-wide clinical programs and initiatives at the satellite site
• Identify operational or clinical issues affecting patient care and communicate concerns to the appropriate CVFHT leadership
• Contribute to maintaining a safe, organized, and patient-centred clinical environment
• Participate in site-specific meetings, huddles, and operational activities as required
• Collaborate with patients, families, physicians, and inter-professional team members in care planning and delivery
• Coordinate care and facilitate referrals to internal and external providers as appropriate
• Liaise with hospitals, community agencies, and other partners to support continuity of care
• Facilitate appropriate communication and information sharing between care providers while maintaining patient confidentiality
• Support patients in navigating the healthcare system and accessing appropriate services and resources
• Promote seamless transitions between primary care, community, hospital, and other healthcare settings

Education, Research & Quality Improvement

• Support a culture of continuous learning and professional development
• Participate in quality improvement initiatives aligned with CVFHT priorities and patient population needs
• Identify opportunities to improve clinical processes, patient access, safety, experience, and outcomes
• Participate in data collection, program evaluation, audits, and other improvement activities as required
• Contribute to research and evaluation activities where appropriate
• Apply evidence-informed practices and current clinical guidelines to nursing care
• Collaborate with leadership to design and implement programs aligned with population health needs and system priorities
• Support development and evaluation of services using patient-centered and co-design approaches
• Participate in program planning, implementation, and evaluation activities
• Contribute to partnerships with community and system stakeholders
• Support change management initiatives and continuous improvement efforts
• Participate in committees, working groups, and organizational initiatives as required
• Perform additional duties as assigned by leadership

Qualifications

Required

• Bachelor of Science in Nursing (BScN) or equivalent nursing education recognized in Ontario.
• Current, unrestricted registration with the College of Nurses of Ontario
• Minimum 3 years of clinical experience in primary care (Family Health Team experience preferred)
• Fluency in both English and French (written and verbal) is considered an asset
• Other assets: experience in chronic disease management, familiarity with electronic medical records (EMR) systems and experience in group facilitation and patient education
• Strong clinical assessment, critical thinking, and decision-making skills.
