This Nursing position is part of the Community Commitment Program for Nurses (CCPN) which provides a $25,000 incentive to eligible nurses in exchange for a 24-month commitment to an eligible employer

What is Pay Per Visit?

• A competitive rate paid per client visit, rather than care code or care time
• Additional payment for visits exceeding 1hr 59 min
• Allows flexibility to work ahead or take needed time
• Relies on your professional judgment for client care
• FT nurses are guaranteed 70 visits biweekly for income stability

Responsibilities

• Complete comprehensive client assessments, develop care plans, and evaluate care based on evidence and client needs
• Serve as primary provider for a caseload, leading care while collaborating with the team
• Support clients through care transitions and connect them with community services

Requirements

• Current registration with the College of Nurses of Ontario
• Optional CNA certification
• Enthusiasm for community care and evidence-based practice
• Current CPR certification
• Valid driver’s license and vehicle
• Updated immunization, criminal background, and Vulnerable Sector checks
• Willingness to attain necessary certifications
• Availability for casual weekends and travel within Huron
• Physical ability to perform demanding tasks
