Job summary

Guelph Community Health Centre is seeking a PSH Support Coordinator to assist vulnerable residents at multiple sites in the community. The role focuses on system navigation, care coordination, and one-to-one support to improve housing stability and access to healthcare.

The position requires a university degree or college diploma in social work or related field, 3–5 years in a community setting, and strong advocacy skills.

Qualifications

  • University degree or college diploma in social work or other relevant discipline.
  • Three to five years’ experience in a community-based organization.
  • Strong interpersonal, active-listening and advocacy skills.
  • Familiarity with local social service and health programs, services, and systems.
  • Experience with clients who experience mental health concerns, substance use, complex chronic diseases, and language barriers.
  • Understanding of harm reduction principles.
  • Excellent English communication; second language is an asset.
  • Proficiency in computers and various software programs.
  • Valid Class G Ontario Driver's License and access to a vehicle (2M liability).

Responsibilities

  • Act as a resource to clients, assisting them in navigating social service and healthcare systems.
  • Assist clients with complex needs to engage in their care plans as outlined by their care team.
  • Engage clients with their primary care providers and community supports.
  • Advocate with health and social service systems/providers to ensure clients receive care.
  • Facilitate referrals to address needs within determinants of health and client goals.
  • Lead case coordination between interdisciplinary team and external partners.

The Permanent Supportive Housing (PSH) Program offers a long-term, affordable housing option combined with flexible, voluntary support services to help high-needs individuals experiencing homelessness maintain stable housing and live independently.

Delivered through partnerships among multiple agencies, the program assists residents of the PSH building who face significant challenges such as substance use, mental health issues, and other complexities. It focuses on helping them achieve housing stability and improve their quality of life by providing access to housing, practical assistance, counseling, advocacy, landlord mediation, and eviction prevention.

As a member of the Allied Health Team, the PSH Support Coordinator works with vulnerable individuals who are residents at the permanent supportive housing building at 10 Shelldale, as well as Grace Gardens, and the transitional housing program at 65 Delhi Street, who are facing multiple and complex barriers to accessing healthcare through system navigation and direct, one-to-one support.

This position works at multiple sites in the community.

Core Responsibilities

  • Actively work to engage clients with their primary care providers and community supports.
  • Advocate with health and social service systems/providers to ensure clients receive the care they deserve.
  • Facilitate internal and external referrals to address needs within the determinants of health to address client goals.
  • Lead case coordination between interdisciplinary team and external partners.
Qualifications
  • University degree or college diploma in social work or other relevant discipline, or an appropriate combination of education and experience.
  • Three to five years’ experience working in a community-based organization.
  • Strong interpersonal, active-listening and advocacy skills.
  • Familiarity with local social service and health programs, services, and systems.
  • Experience working with clients who experience mental health concerns, use substances,havecomplex chronic diseases, and language barriers.
  • Understanding of, and alignment with, the philosophy of harm reduction.
  • Excellence in English written and oral communication.Second language is an asset.
  • Proficiency in the use of computers and various software programs.
  • Possession of a valid Class \"G\" Ontario Driver's License and access to a vehicle. $2 million in liability coverage will be required for your vehicle.
