Job summary

ComForCare Home Care in Ontario is seeking a Registered Nurse (RN) or Registered Practical Nurse (RPN) to conduct client assessments, create personalized care plans, implement interventions, and coordinate referrals to the health team. You will maintain detailed records and ensure client safety at home.

Ideal candidates have a G license, own insured vehicle, strong assessment and communication skills, and the ability to work independently with compassion.

Qualifications

• Current registration as RN or RPN in Ontario.
• Strong assessment and documentation skills.
• Experience in home care, community health, geriatrics or related areas required.
• Compassionate, client-focused and able to work independently.
• G license and own insured vehicle.
• Maintains confidentiality and knowledge of safety/infection control.

Responsibilities

• Perform comprehensive assessments of client status (physical, psychosocial, environmental).
• Develop individualized care plans based on data analysis and current evidence.
• Provide interventions to achieve realistic client outcomes.
• Report changes to update the plan of care as progress occurs.
• Administer medications as needed.
• Maintain client records with assessments, planning and evaluations.
• Initiate referrals to other health team members.

What we are looking for:

What you will be doing:

• Providing interventions with a focus on achieving realistic client outcomes.
• Reporting changes to update the plan of care to reflect progress towards goals and outcomes.
• Administering medications as needed.
• Maintaining client records showing systematic assessments, planning interventions, and evaluations.
• Initiating referrals as needed to other health team members.

Pay Range: $50 per intake
