Job summary

Russell Tobin is seeking a compassionate Registered Nurse to provide home and community-based care across British Columbia. You will perform comprehensive patient assessments, develop individualized care plans, and deliver skilled nursing interventions in patients’ homes and communities.

Qualifications

  • Active RN registration in good standing with BCCNM.
  • Home care nursing experience preferred.
  • Experience with CMOIS and Pixelere systems preferred.
  • Trained in Home Care Nursing practices.
  • Training in Wound Care, IV Therapy, and Palliative Care.
  • Current CPR/BLS certification.
  • Strong clinical assessment and care coordination skills.

Responsibilities

  • Conduct comprehensive patient assessments and ongoing evaluations within home care settings.
  • Develop, implement, and monitor individualized care plans.
  • Deliver skilled nursing care according to patient needs and treatment plans.
  • Provide wound care management including advanced therapies and wound vac care.
  • Administer IV therapy and monitor treatment effectiveness.
  • Support palliative care patients and families with compassionate care and education.
  • Collaborate with physicians and multidisciplinary teams.
  • Provide education and support to patients and caregivers.
  • Maintain accurate and timely clinical documentation using electronic systems.
