Alberta Health Services in Claresholm, Alberta, is seeking a Registered Nurse to join an inter-professional team that links Rural Home Care clients with essential health and community services. Using a person-centered approach, you will develop care plans, support families, and provide direct nursing care across the life span.
You will work in the Rural South/Claresholm area, with shifts days, evenings, and weekends.

Based on the Continuing Care Case Management Competency Framework you demonstrate ability in the areas of personal and professional accountability, systems thinking, facilitating creative thinking, communicating and building relationships. Using a person-centered approach, you will develop a care plan supporting the client/family to identify needs, focus on strengths, set achievable personal health and self-care goals and provide direct nursing care to individuals across the life span. In collaboration with the client/family you will ensure services are provided and modified on an ongoing basis, based on client’s response to interventions, incorporating stewardship into service authorization.
Please note that this position supports the communities 130km SE of the Calgary City Limits and or 80km NE of the City Limits of Lethbridge. The Rural South team is comprised of Diamond Valley, Okotoks, High River, Nanton, Claresholm and Vulcan. The farthest site is 160km south of the city. “This position may qualify for Rural Capacity Investment Fund (Up to $15,000/year (rural) or $20,000/year (remote), 1- or 2-year agreement”.
As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values. PLEASE NOTE: Provisional Permit holders will be paid $40.76 to $55.79 per hour. Upon obtaining a full practice permit, the rate of pay will be adjusted to the applicable Registered Nurse rate in accordance with UNA Article 25.02(b)(i) or (ii), as applicable.
