Job summary

Qualifications

Registered Dietitian; eligible for membership with the College of Dietitians of Ontario.
Extensive knowledge of geriatric nutrition.
Proficiency with computer applications (MS Office); experience with RAI MDS 2.0 desirable.

Responsibilities

Consult with and advise the Food Service Manager and Director of Care on nutrition and hydration needs.
Perform nutritional assessments for residents and develop individualized care plans.
Develop, implement and evaluate nutritional care plans and monitor progress.
Maintain communication with the Food Service Manager regarding changing resident needs.
Review seasonal menus for nutritional adequacy and provide written approvals.
Educate residents, families, and team members on nutrition and hydration recommendations.
Participate in interdisciplinary teams and committees related to resident care and facility operations.
