Reporting to the Lead Physician, the Dietitian will provide nutrition assessments, individualized counselling, care planning, and education, primarily supporting older adults and individuals living with chronic health conditions. The successful candidate will collaborate closely with physicians, registered nurses, and other healthcare professionals to promote healthy lifestyles, improve patient outcomes, and support effective chronic disease management.

This position is part of a newly developed Innovation Fund initiative between the Government of Saskatchewan and medical clinics across the province. The position is temporary until March 31, 2028, with the possibility of extension, subject to funding availability and operational needs.

If you have a degree in Nutrition and Dietetics and are registered, or eligible for registration, with the Saskatchewan College of Dietitians, this opportunity may be for you!

Responsibilities:
  • Conduct comprehensive nutrition assessments and develop individualized nutrition care plans based on patients' medical histories, nutritional needs, dietary habits, and health goals.
  • Provide evidence-based nutrition counselling and education, particularly for older adults and individuals managing chronic conditions such as diabetes, cardiovascular disease, and hypertension.
  • Monitor patient progress, evaluate treatment outcomes, and modify nutrition interventions as needed.
  • Deliver individual and group nutrition education sessions to promote healthy eating, disease prevention, and effective chronic disease management.
  • Collaborate with physicians, registered nurses, and other healthcare professionals in multidisciplinary care planning, case discussions, and patient follow-up.
  • Communicate nutrition recommendations and relevant patient concerns clearly to patients, families, caregivers, and members of the healthcare team.
  • Adapt communication and education approaches to meet the needs of older adults and patients with varying health, cognitive, hearing, or communication needs.
  • Maintain accurate, timely, and confidential patient records, assessments, care plans, and follow-up notes using the clinic's electronic medical record (EMR) system.
  • Develop, deliver, and evaluate nutrition programs, educational resources, and community-based health promotion initiatives to meet patient and community needs.
  • Participate in quality improvement initiatives and contribute to the ongoing improvement of nutrition services and patient care processes.
  • Maintain current knowledge of evidence-based dietetic practices, geriatric nutrition, and chronic disease management.
  • Ensure compliance with applicable legislation, professional standards, privacy requirements, and clinic policies.
  • Perform other related duties as assigned to support clinic operations and patient care.
Qualifications:
  • Bachelor of Science in Nutrition and Dietetics from an accredited program.
  • Current registration, or eligibility for registration, with the Saskatchewan College of Dietitians.
  • Registration or membership with Dietitians of Canada, where applicable to the position.
  • Valid Saskatchewan Class 5 driver's licence.
  • Strong clinical assessment, analytical, and decision-making skills, with the ability to develop appropriate, evidence-based nutrition care plans.
  • Excellent oral and written communication skills, with the ability to explain nutrition information clearly and effectively to patients and healthcare professionals.
  • Strong organizational and time management skills, with the ability to manage competing priorities and adapt to changing patient and clinic schedules.
  • Proficiency in electronic medical records (EMR) and computer applications, including Microsoft Word, Excel, and Outlook.
  • Ability to work independently and collaboratively within a multidisciplinary healthcare team.
  • Demonstrated initiative, accountability, confidentiality, and commitment to patient-centred care.
  • Experience working with older adults and individuals with chronic health conditions is strongly preferred.
  • Experience in community dietetics, primary care, family medicine, community health, or long-term care is considered an asset.
  • Experience developing and delivering individual or group nutrition education programs is an asset.

If you are passionate about improving patient health through evidence-based nutrition care and collaborative healthcare delivery, this opportunity at Living Spring Medical Centre may be the right fit for you!
